My Survey

This page will be linked to your Home Page using one of the buttons you designed in that assignment.

Make note of the time line and dues dates ~ they must be adhered to for full credit potential

Preparation for this assignment includes:

  1. Compose a survey
    1. Choose a topic from which you can create a survey.
    2. Get the teacher's approval of your chosen topic.
    3. Create five questions for the survey.  The questions need to be phrased in a manner so that the response would be multiple choice or a one word fill-in-the-box type answer.
Following are the requirements for this page:
  1. Create/write the HTML page in _______.  Use asp as your "driver" for survey data collection.
  2. On your Web page include the following:
    1. A form.  This form will be used in the collection of survey responses.  Specific form content requirements will be provided in class.
    2. The survey questions with their appropriate response options.
    3. A disclaimer about the survey stating its confidentiality and purpose (educational only); no names will be released or posted elsewhere
  3. Create a database in Access which corresponds to the fields in your form.  Save the database as YourNameSurvey.mdb
  4. Test the survey to ensure data is recorded appropriately.
  5. Save the page as mysurvey.htm.
  6. Print the HTML from ______ and the Table view of the database on the classroom printer (CC105 LaserJet).
Once the page is graded and, if necessary, corrected, we will FTP the page and the database to the Web server.

The survey will be executed in several classes so that you have data to analyze.  Once the survey process is complete, we will download the Access database back off the server and analyze the results.
